The challenge
A growing cohort, narrow dosing rules and monitoring that slips
More patients, more complexity
DOAC prescribing continues to rise, with more older, frailer and multimorbid patients on treatment, often alongside interacting medicines.
Dosing depends on moving targets
Correct dosing relies on renal function, weight, age and indication. When these change and the dose does not, patients are exposed to bleeding or thromboembolic risk.
Monitoring and recall fall behind
Renal function, full blood count and liver function checks are due at least annually and more often as renal function falls. Without a reliable recall process, overdue monitoring builds up unseen.
The service
From healthcheck to a monitored, recalled cohort
- 01
DOAC healthcheck
We validate your DOAC register, check coding, and run a structured search across indication, dose, renal function, weight, age, interacting medicines and monitoring status.
- 02
Detailed audit report
You receive a clear written report: cohort size, patients on an incorrect or unverified dose, overdue or missing monitoring, high-risk interactions, and a prioritised action list.
- 03
Risk-stratified patient reviews
Our pharmacist prescribers review patients in priority order to optimise prescribing, confirm accurate dosing, and make sure monitoring is clinically appropriate, safe and up to date. Changes are agreed with the patient, actioned and coded in your clinical system.
- 04
Tools, processes and handover
We set up searches, recall templates, a monitoring protocol and a standard operating procedure so your team can keep the cohort safe after we hand over, with optional ongoing monitoring support.
