Polypharmacy and Deprescribing
We provide structured, safe deprescribing. We identify problematic polypharmacy, carry out patient-centred reviews, run planned opioid and benzodiazepine reduction programmes with follow-up, and document every decision in line with national guidance.

Outcomes we work towards
Fewer medicines-related admissions and falls
Reduced dependence-forming prescribing
Lower prescribing spend
Governance
Deprescribing follows national guidance on medicines associated with dependence and is agreed with the patient. Plans are documented and shared with the responsible GP.
